Texas Breast Health Community Awards

These Awards will be presented each year by the Breast Health Collaborative of Texas to honor and recognize those individuals, groups, institutions or non-profits of the Texas Breast Health Community whose works and achievements ensure access to quality breast health care for all Texans.

 

 

 

EXTRAORDINARY CONTRIBUTIONS

To to honor an individual who has made outstanding contributions and extraordinary strides in breast health including research, academic or educational, or philanthropic contributions and extraordinary strides toward the understanding and eradication of  Breast Cancer.

 

 

 

 

 

 

CHW BREAST HEALTH ACHIEVEMENT

 

To honor an individual Community Health Worker who has demonstrated extraordinary excellence in Community breast health education, program development or provision of vital community breast health service.

 

 

 

 

 

 

BREAST CANCER ADVOCATE

 

To honor an individual who has demonstrated dedication and resolve in the fight for equity and health for all breast cancer patients.

 

 

 

 

 

 

OUTSTANDING ACHIEVEMENT by a TEAM or NON-PROFIT

This award recognizes a nonprofit or organizational team who has demonstrated extraordinary and innovative initiatives to implement change within the breast health landscape.

 

 

 

 

 

 

BASIC ELIGIBILITY

 These awards will be based on the individual’s accomplishments, dedication, leadership, length of service, and impact on the breast health community. 

 

The winner must be present to accept the award at the Annual Breast Health Summit Awards Dinner.

Nominations

The application requires the following:

  • Nominee’s contact information

  • Nominator’s contact information

  • A short explaination of the following:

    • Who the individual is

    • What their accomplishments are

    • Length of service

    • How they have impacted the breast health arena

    • What makes them extraordinary

  • Indication that the nominator has notified the nominee that their name has been submitted

  • Indication that the nominee understands that they must be present to accept the award
